Chemical peel is a non-invasive cosmetic procedure, where a chemical solution is applied to the skin, causing it to exfoliate and eventually peel off. The procedure removes old, damaged skin cells, and after peeling off, newer, healthier, and more youthful skin appears with fewer wrinkles and lines. The procedure is not limited only to the face but also used on the neck and hands. It has undergone clinical tests and is FDA-approved for the treatment of wrinkles, freckles, melasma, rosacea, acne, and dark spots on the skin.

There are 3 types of chemical peels, depending on the client’s needs. Mild or superficial peels use mild acid solutions, such as alpha-hydroxy acid (AHA). It only penetrates the outermost layers of the skin, gently exfoliating it and improving its appearance. A moderate or medium peel uses slightly stronger acids such as trichloroacetic or glycolic acid to reach the middle skin layer, improving any discoloration and scurf it out. Deep peels use phenol acid to completely remove damaged skin cells, removing the wrinkles, freckles, and lines.

Before the procedure is done, our clients consult with the doctors on their problems and expectations. At the clinic, we perform the procedure on an outpatient basis. The face is cleaned and protective gauze or googles are used to cover the eyes. The area under treatment is also restricted to keep the acid within it. Typical anesthesia is used to numb the skin. The process used in each of the three types of peels varies slightly. With superficial peel, the doctor uses a cotton ball or a brush to apply salicylic acid on the skin, which causes the skin to whiten. One may feel a slight stinging sensation. Afterward, the doctor cleans off the skin with water and a neutralizing solution is applied.

For moderate peel, a cotton ball or brush is used to apply trichloroacetic acid and the skin begins to whiten and a stinging sensation is also felt. After some time, the doctor smears a cool compress on the skin and waits for about 20 minutes. No neutralizing solution is needed. The deep peel however is more drastic as one is given intravenous (IV) fluids and the heart rate is monitored as a precaution. The doctor applies phenol acid and the skin turns white. The procedure is done at intervals of 15 minutes each for about 90 minutes. This is done to reduce the exposure time to phenol acid.

Chemical peels are extremely effective and the results are amazing. The skin texture improves and reduces the appearance of wrinkles. Results are seen after a period of about 1-2 weeks for mild and moderate peels and up to 3 weeks for deep peels. It is neither permanent nor a one-time treatment as one needs to schedule subsequent visits of about 3-5 sessions, within 2-5 weeks after the previous treatment.

Not everyone is free to have chemical peels as it is not feasible for pregnant and lactating mothers and patients with frequent outbreaks of cold sores, keloid patients (have an overgrowth of scar tissues), and patients under isotretinoin medication for acne. Fairly skinned and fairly-haired individuals are better candidates. Dark-skinned people are also viable but one may risk uneven skin pigmentation and uneven skin tone after the procedure.

The side effects include persistent redness, scarring, burning, or bruising due to the acids used. It may also cause bacterial or fungal infections such as herpes or cold sores. One may also develop hypopigmentation (from the deep peels) or hyperpigmentation (from mild peels). Deep chemical peels may cause heart, kidney, or liver damage due to the use of phenol acid. The skin is extra sensitive to sunlight after the procedure, hence due care needs to be taken to protect it from sunlight.
